Step 1: Visit http://www.ieee.org/organizations/rab/md/smforms.htm. Take a look at both forms.
Step 2: Locate two senior members to serves as references. You can nearly always find them at the dinner meetings. You need two references, and you do not need their IEEE membership numbers. They enter that information in the nomination form.
Step 3: Provide your nominators with the information asked for in the nomination form, paying special attention to the information describing significant accomplishments you have made (your resume does not substitute for that).
Please note: If you have problems locating a Senior Member as a reference, contact Mark Lamendola (writer @ieee.org). In exchange for your agreeing to be the Senior Member Coordinator for one year (pending approval by the Section Chair), Mark will provide you with contacts. Once you are the SM Coordinator, you will have access to IEEE's list of Senior Members. The SM Coordinator job isn't time-consuming.
